Disposition of Your Remains

The last session of the Wisconsin State Legislature passed a law that allows a living individual to direct how his/her remains should be disposed of at the time of death.  In the past, this was left up to "next of kin" and occasionally resulted in family squabbles.  In an effort to address this problem, the Legislature has authorized a form called "Authorization for Final Disposition" which allows you to name a person who can make the decision for you.  It also allows you to give instructions to that individual as to your wishes as to disposition (burial, cremation, etc.), arrangements for viewing, religious services and even suggested sources of funds to pay for these matters.  You can find this form, along with the instructions, at http://dhs.wisconsin.gov/forms/advdirectives/f00086.pdf .
